Our Notice of Privacy Practices describes how medical information about your child may be used and disclosed, and how you can get access to this information, as required by the Health Insurance Portability and Accountability Act (HIPAA). Please review it carefully.

• Ask us to correct your child’s medical record
• Ask us to contact you in a confidential way
• Ask us to limit what we use or share
• Get a list of those with whom we’ve shared information
• Get a copy of the privacy notice
• Choose someone to act for you
